Job Shop Lean An Industrial Engineering Approach to Implementing Lean in High-Mix Low-Volume Production Systems

Job Shop Lean An Industrial Engineering Approach to Implementing Lean in High-Mix Low-Volume Production Systems

In the 1950’s the design and implementation of the Toyota Production System (TPS) within Toyota had begun. In the 1960’s Group Technology (GT) and Cellular Manufacturing (CM) were used by Serck Audco Valves a high-mix low-volume (HMLV) manufacturer in the United Kingdom to guide enterprise-wide transformation. In 1996 the publication of the book Lean Thinking introduced the entire world to Lean. Job Shop Lean integrates Lean with GT and CM by using the five Principles of Lean to guide its implementation: (1) identify value (2) map the value stream (3) create flow (4) establish pull and (5) seek perfection. Unfortunately the tools typically used to implement the Principles of Lean are incapable of solving the three Industrial Engineering problems that HMLV manufacturers face when implementing Lean: (1) finding the product families in a product mix with hundreds of different products (2) designing a flexible factory layout that fits hundreds of different product routings and (3) scheduling a multi-product multi-machine production system subject to finite capacity constraints. Based on the Author’s 20+ years of learning teaching researching and implementing Job Shop Lean since 1999 this book Describes the concepts tools software implementation methodology and barriers to successful implementation of Lean in HMLV production systems Utilizes Production Flow Analysis instead of Value Stream Mapping to eliminate waste in different levels of any HMLV manufacturing enterprise Solves the three Industrial Engineering problems that were mentioned earlier using software like PFAST (Production Flow Analysis and Simplification Toolkit) Sgetti and Schedlyzer Explains how the one-at-a-time implementation of manufacturing cells constitutes a long-term strategy for Continuous Improvement Explains how product families and manufacturing cells are the basis for implementing flexible automation machine monitoring virtual cells Manufacturing Execution Systems and other elements of Industry 4. 0 Teaches a new method Value Network Mapping to visualize large multi-product multi-machine production systems whose Value Streams share many processes Includes real success stories of Job Shop Lean implementation in a variety of production systems such as a forge shop a machine shop a fabrication facility and a shipping department Encourages any HMLV manufacturer planning to implement Job Shop Lean to leverage the co-curricular and extracurricular programs of an Industrial Engineering department | Job Shop Lean An Industrial Engineering Approach to Implementing Lean in High-Mix Low-Volume Production Systems

Lean Healthcare Systems Engineering for Clinical Environments A Step-by-Step Process for Managing Workflow and Care Improvement Projects

Lean Healthcare Systems Engineering for Clinical Environments A Step-by-Step Process for Managing Workflow and Care Improvement Projects

It has been almost 20 years since the Institute of Medicine released the seminal report titled Crossing the Quality Chasm. In it the IoM identified six domains of care quality (safe timely effective efficient equitable and patient-centric) and noted a huge gap between the current state and the desired state. Although this report received a great deal of attention sadly there has been little progress in these areas. In the U. S. healthcare still has huge disparities is inefficient and is fragmented with delays in care that are often unsafe. Most U. S. citizens are expected to suffer from a diagnostic error sometime during their lifetime not receive a large fraction of recommended care and pay for one of the most expensive systems in the world. Much has been written about quality improvement over the years but many prominent quality and safety experts. Yet progress has been slow. Some have called on the healthcare professions to look outside of healthcare to other industries using examples in nuclear power and airlines for safety the hotel and entertainment industry for a ‘customer’ focus and the automotive industry particularly Toyota for efficiency (Lean). This book by Dr. Oppenheim on lean healthcare systems engineering (LHSE) is a fresh approach that brings forth concepts that systems engineers have used in huge national defense projects. What’s unique in this book is that these powerful system engineering tools are modified to be able to address smaller sized healthcare problems that still involve similar problems in fragmentation and poor communication and coordination. This book is an invaluable reference for a new powerful process named Lean Healthcare Systems Engineering (LHSE) for managing workflow and care improvement projects in all clinical environments. The book applies to ambulatory clinics and hospitals of all types including operating rooms emergency departments and ancillary departments clinical and imaging laboratories pharmacies and population health. The book presents a generic rigorous but not mathematical step-by-step process of integrated healthcare systems engineering and Lean. The book also contains the first major product created with the LHSE process namely tabularized summaries of representative projects in healthcare delivery applications called Lean Enablers for Healthcare Projects. Each full-page enabler table lists the challenges and wastes powerful improvement goals risks and expected benefits and some useful descriptions of the healthcare system of interest. The book provides user-friendly solutions to major problems in healthcare delivery operations in all clinical environments addressing fragmentation wastes wrong incentives ad-hoc and stove-piped management lack of optimized processes hierarchy gradient lack of systems thinking “blaming and shaming culture” burnout of providers and many others. | Lean Healthcare Systems Engineering for Clinical Environments A Step-by-Step Process for Managing Workflow and Care Improvement Projects

Who Knew? Inside the Complexity of American Health Care

Who Knew? Inside the Complexity of American Health Care

Despite all the writing and the research America’s health care industry continues to fail at providing health care that is accessible and affordable with measurable quality. The fundamental reason we have failed is that health care is not only a complex business but the most complex in our economy. Other industries are disrupted some readily adapt to new markets; some leverage information technology and innovative and cost-saving ways. But to date health care has resisted. The customary approaches tried in other industries seem not to apply to health care. Why? Why is the health care industry so politically divisive? Why is the quality of health care services so difficult to measure? Why do patients often fail to understand their own health care? Why are security and privacy such unique challenges in health care? Why is the payment process for health care services so complicated and challenging? This book seeks to answer these questions. This book written by a well know industry ‘insider’ with 35+ years working at senior levels in hospital operations and information technology discusses nine major factors that in combination contribute to health care’s complexity. The author concludes that until we understand why health is so complex we will continue to see books complaining about the poor state of health care in the U. S. and proposals for change that are generally unsuccessful and innovative technology products that fail to deliver expected results. | Who Knew? Inside the Complexity of American Health Care

The Resilient Healthcare Organization How to Reduce Physician and Healthcare Worker Burnout

The Resilient Healthcare Organization How to Reduce Physician and Healthcare Worker Burnout

Professional burnout is an epidemic in America. Approximately half of physicians and nurses are affected and at risk for themselves and their patients. Much has been written about professional burnout. The term was originally coined in the 1970s by American psychologist Herbert Freudenberger to describe the consequences of severe stress and high ideals experienced by people working in helping professions. Since then many books have been written to address this looming national public health crisis. But unfortunately there has been much less written from a solution standpoint: getting to the root cause of why this is occurring now more than ever. The Resilient Healthcare Organization engages readers focusing on physicians and healthcare professionals and their experiences and how they overcame a loss of enthusiasm for work feelings of cynicism and a low sense of personal accomplishment. The feelings of emotional exhaustion are characterized by depersonalization and perceived ineffectiveness. These are the cardinal features that define burnout and affect almost 50% of physicians and 30–70% of nurses. This book addresses why burnout is viewed as a threat and how it can be fought. The author discusses the contributing factors and solutions at the health system and societal level. Additionally this book explores the current and future etiology and impacts on physicians and healthcare professionals with a significant emphasis on solutions at both the individual level and the system level. According to the U. S. Bureau of Labor Statistics some of the fastest projected job growth will occur in the healthcare healthcare support and personal care fields which accounts for almost one third of the total employment growth by 2020. Analysts suggest that the healthcare field is growing because of the long-term trends of an aging population and improved diagnosing and treatment options. Strategy-Driven Leadership The Playbook for Developing Your Next Generation of Leaders

Strategy-Driven Leadership The Playbook for Developing Your Next Generation of Leaders

It’s estimated that U. S. companies spend over $14 billion annually on leadership development -Match that number to the abundant and growing research that finds most leadership development to be ineffective and the conclusion is a phenomenal amount of waste. The remedy for this situation is to have business strategy drive leadership development instead of creating programs that match a one-size-fits-all approach to leadership. This book's approach called Strategy-Driven Leadership Development (SDLD) puts business strategy first. It maintains an emphasis on building leadership programs around what it will take to make the business successful as opposed to implementing a program in the hopes that it will benefit the strategy. The result is a differentiated and targeted approach called Intentional Leadership Development which provides the structure for transforming how leadership development is undertaken. At the heart of this book however is the explanation of how small incremental changes in action and perspective create meaningful changes in the way leadership is developed. The focus is on the leadership behaviors associated with success for any company. Some companies may need leaders with better financial acumen while others may require better teamwork for success. These skills are learnable and when the energy of an organization is behind it then leadership development can be transformational. The authors method retools prior leadership efforts – the emphasis is not on previous failures and restarting with new programs. There are many effective ideas and actions that are currently embedded in leadership programs but they miss the critical element of tying their efforts to the business strategy. Strategy-Driven Leadership changes the way organizations think about and drive their leadership talent initiatives among their current and upcoming leaders. The book is filled with research science-based information case studies and practical hands-on tools on why and how this Strategy-Driven Leadership Development model will transform company leadership approaches. | Strategy-Driven Leadership The Playbook for Developing Your Next Generation of Leaders

Digital Health and Patient Data Empowering Patients in the Healthcare Ecosystem

Digital Health and Patient Data Empowering Patients in the Healthcare Ecosystem

Patients with unmet needs will continue to increase as no viable nor adequate treatment exists. Meanwhile healthcare systems are struggling to cope with the rise of patients with chronic diseases the ageing population and the increasing cost of drugs. What if there is a faster and less expensive way to provide better care for patients using the right digital solutions and transforming the growing volumes of health data into insights? The increase of digital health has grown exponentially in the last few years. Why is there a slow uptake of these new digital solutions in the healthcare and pharmaceutical industries? One of the key reasons is that patients are often left out of the innovation process. Their data are used without their knowledge solutions designed for them are developed without their input and healthcare professionals refuse their expertise. This book explores what it means to empower patients in a digital world and how this empowerment will bridge the gap between science technology and patients. All these components need to co-exist to bring value not only to the patients themselves but to improve the healthcare ecosystem. Patients have taken matters into their own hands. Some are equipped with the latest wearables and applications engaged in improving their health using data empowered to make informed decisions and ultimately are experts in their disease(s). They are the e-patients. The other side of the spectrum are patients with minimal digital literacy but equally willing to donate their data for the purpose of research. Finding the right balance when using digital health solutions becomes as critical as the need to develop a disease-specific solution. For the first time the authors look at healthcare and technologies through the lens of patients and physicians via surveys and interviews in order to understand their perspective on digital health analyse the benefits for them explore how they can actively engage in the innovation process and identify the threats and opportunities the large volumes of data create by digitizing healthcare. Are patients truly ready to know everything about their health? What is the value of their data? How can other stakeholders join the patient empowerment movement? This unique perspective will help us re-design the future of healthcare - an industry in desperate need for a change. | Digital Health and Patient Data Empowering Patients in the Healthcare Ecosystem

Transforming Mental Healthcare Applying Performance Improvement Methods to Mental Healthcare

Transforming Mental Healthcare Applying Performance Improvement Methods to Mental Healthcare

One in five U. S. adults experiences a mental illness within a given year. With more than 550 000 people working to support this underserved community the mental healthcare system has grappled with inadequacies and shortcomings in safety quality and care delivery. There is a wide range of problems from access-to-care issues and errors to complications stemming from poor care. Our country is also on an unsustainable path as our healthcare expenditure keeps growing. To add to all of this we are facing a rampant epidemic of burnout among healthcare workers. Modern advancements introduced with many promises—such as electronic medical records newer medications or advanced treatments—have created unique challenges when ushered into a highly regulated healthcare system. What does it take to provide patients with everything they need—the right quality of care at the right time and at the right cost—to keep them healthy? Which process steps add value? Which steps are wasteful? A widely accepted fact is that a conservative 30-50% of every step in the mental healthcare process does not help patients feel better or stay better. When considering delays in care workarounds excessive documentation and an overuse of auditing the care system has moved highly skilled clinicians away from providing value as administrative tasks continue to encroach on their time. There is a clear need to rethink and redesign the system of care. This book is a primer for understanding the current state of the mental health system and the performance improvement skills and leadership acumen needed to address existing challenges. Sheppard Pratt the award-winning leading institution for mental healthcare in America provided the focus on mental healthcare and became the laboratory for this body of work over the course of eight years. It hired a seasoned systems thinker with improvement expertise to work with mental health professionals and solve some of their most complex and chronic problems. The book is a result of the collaboration between a practicing psychiatrist in a leadership role and the systems engineer. Working together they demonstrate how to think about redesigning care and redefining the nature of work to enhance value for both the people served and the healthcare workforce. They crafted a multi-pronged approach towards culture change at Sheppard Pratt including implementing a course on Learning to Improve which introduced staff to a performance improvement methodology. There are several vignettes interwoven throughout the book that describe the complexities and constraints of the system. Solving some of these challenges creates a new paradigm of work while minimizing waste and enhancing value. | Transforming Mental Healthcare Applying Performance Improvement Methods to Mental Healthcare

My personal mission is to create systemic change with an impact on the global stage. This playbook needs to be on the desk of every executive clinician and patient today. -Mahek Shah MD Senior Researcher and Senior Project Leader Harvard Business SchoolOur current healthcare system’s broken. The Organization for Economic Co-Operation and Development (OECD) predicts health care costs could increase from 6% to 14% of GDP by 2060. The cause of this increase is due to (1) a global aging population (2) growing affluence (3) rise in chronic diseases and (4) better-informed patients; all of which raises the demand for healthcare. In 2006 Michael Porter and Elizabeth Teisberg authored the book ‘Redefining Health Care: Creating Value-Based Competition on Results. ’ In it they present their analysis of the root causes plaguing the health care industry and make the case for why providers suppliers consumers and employers should move towards a patient-centric approach that optimizes value for patients. According to Porter value for patients should be the overarching principle for our broken system. Since 2006 Professor Porter accompanied by his esteemed Harvard colleague Profesor Robert Kaplan have worked tirelessly to promote this new approach and pilot it with leading healthcare delivery organizations like Cleveland Clinic Mayo Clinic MD Anderson and U. S. Department of Veteran Affairs. Given the current state of global healthcare there is urgency to achieve widespread adoption of this new approach. The intent of this book is to equip all healthcare delivery organizations with a guide for putting the value-based concept into practice. This book defines the practice of value-based health care as Value Management. The book explores Profesor Porter’s Value Equation (Value = Outcomes/ Cost) which is central to Value Management and provides a step-by-step process for how to calculate the components of this equation. On the outcomes side the book presents the Value Realization Framework which translates organizational mission and strategy into a comprehensive set of performance measures and contextualizes the measures for healthcare delivery. The Value Realization Framework is based on Professor Kaplan's ground-breaking Balanced Scorecard approach but specific to healthcare organizations. On the costs side the book details the Harvard endorsed time-driven activity based costing (TDABC) methodology which has proven to be a modern catalyst for defining HDO costs. Finally this book covers the need and a plan to establish a Value Management Office to lead the delivery transformation and govern operations. This book is designed in a format where any organization can read it and acquire the fundamentals and methodologies of Value Management. It is intended for healthcare delivery organizations in need of learning the specifics of achieving the implementation of value-based healthcare. | Value Management in Healthcare How to Establish a Value Management Office to Support Value-Based Outcomes in Healthcare
